Please use this form to let us know about your child’s attendance.
You can report an absence, a late arrival, or an early pickup.

Doctor’s notes can be uploaded to Magus Health via the FACTS Family Portal.

If you're reporting for more than one child, no need to fill this out twice—you can include them all here.

You’ll receive a daily email notification any time your child is absent.
