Early Childhood Student Records

These six forms listed below are REQUIRED by Pinellas County for all children 4 and under (as of Sept 1st of the current school year).

Please complete and upload via the Document Upload button below. If you would like paper copies, they can be picked up at the school.

  1. Child’s Enrollment Record

  2. Food Experience Form

  3. Distracted Adult

  4. Flu Brochure

  5. Know Your Child’s Children’s Care Center brochure

  6. Emergency Medical Release

    • Note: Must be notarized. The school office can notarize by appointment (to ensure the staff member needed is onsite).


Documents 1-5 should be downloaded and printed, wet signed with ink, scanned or a good-quality photo taken, and then uploaded to School using the button below.

Document 6 can be filled out at School and notarized there.

In addition to these forms, student records must be complete with the following documents.

These are required for ALL New Students:

  1. Current Student Physical/Health Form

  2. Current Immunization Records or Waiver

  3. Birth Certificate
